Health Leads (formerly Project HEALTH)

Health Leads' mission is to create a health care system in which those unmet resource needs that compromise health are addressed as a standard element of patient care.

In the urban hospitals and health centers where Health Leads operates, physicians can "prescribe" food, housing, job training, fuel assistance, health insurance, or other resources for their low-income patients as routinely as they do medication. Located in the clinic waiting room and staffed by college volunteers, Health Leads "fills" these prescriptions by connecting patients with key community resources. Currently, Health Leads' corps of nearly 700 volunteers staffs 22 Health Leads programs serving over 6,000 families in six cities, including Baltimore, Boston, Chicago, New York City, Providence, and Washington, D.C.
